  • TRANSCRIPT REQUEST FORM

    The transcript request form is for current or former students seeking official or unofficial copies of MBHS transcripts.

  • DIPLOMA NAME (SENIORS)

    SENIORS: Your Diploma Name will be your legal name in PowerSchool. If you would like to make changes, or if your name includes special accents/punctuation, then please complete this form.

  • WORK PERMIT Request Form

    All minors under age 18 must have a work permit. Return completed form to Front Office Staff (must include SSN). Work Permits will be ready for pickup within 24-48 hours. NOTE: Minors who have high school diplomas or have passed the CHSPE are exempt.

  • Parking Permit

    Students who drive need a permit to park on campus. Completed forms shall be returned to the Front Office Receptionist.

  • Dance/Activity Guest Pass

    Activity Guest Passes may be picked up at the Front Office. Completed passes shall be returned to the Assistant Principal's Office for approval.

  • 2017 Prom Contract

    Please fill out your Prom contract and return to Annette Bunnell in the main office before you purchase your tickets.

    Almost all male U.S. citizens and male immigrants, who are 18 through 25, are required to register with Selective Service.
